A group of people selected by an individual with developmental disabilities or other types of disabilities that meets regularly with the individual to help plan, design and support ways for that person to achieve his or her personal goals. Circles are based on the belief that the community is a place where everyone belongs. A circle can include friends, family, classmates, co-workers, professionals, and other community members.
Person-centred planning is designed to assist someone to plan their life and supports. It encourages individual’s personal self-determination and improves their own independence and confidence. As everyone has their own aspirations and dreams for their future, a person centred plan is planned out in different steps for people to achieve them. It doesn't matter how big or small the dream is, it’s the steps that the individual can achieve to get to the big dream which matters and person centred plans can help achieve them.
